Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will visit families at their homes across Virginia, guiding them through Medicaid Waiver programs and helping them live safe, healthy, independent lives.

The role emphasizes compassion, organization, and community focus, with requirements including local travel, strong communication, and a heart for service. A degree is not required, and we value experience and reliability.

Qualifications

• No degree required; heart and experience matter more than formal education.
• At least 2 years of experience supporting individuals with disabilities or the elderly.
• Proficient with email, documents, spreadsheets, and portal navigation.

Responsibilities

• Visit clients in their homes locally.
• Guide families through Virginia Medicaid Waiver programs.
• Write plans of care and assessments.
• Troubleshoot authorizations, timesheets, and related tasks.
• Build lasting relationships with families based on respect and trust.
• Work remotely from a laptop/tablet when needed.
