Job summary

Moms In Motion, a Virginia service facilitation provider, is seeking Service Facilitators to visit clients at home and guide families through Medicaid Waiver programs. You will craft plans of care, assess needs, and resolve issues like authorizations and timesheets, while building lasting relationships with families.

Must live in VA and have a valid driver's license; 2+ years experience with disabilities or elderly populations preferred, no degree required.

Qualifications

• 2+ years experience supporting individuals with disabilities or the elderly.
• No degree required; focus on heart and experience.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle required.
• Strong written and verbal communication; pass background check with references.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Guide families through VA waiver programs and plan care.
• Write care plans and assessments that make a difference.
• Troubleshoot authorizations, timesheets, and related tasks.
• Build trusting relationships with families; work from laptop/tablet.
