Job summary

Moms In Motion in Virginia is seeking a Service Facilitator (SF) Case Manager who will visit families in their homes, guiding them through Virginia’s Medicaid Waiver programs and helping members live safe, healthy, independent lives. The role requires at least 2 years of experience supporting individuals with disabilities or the elderly; no degree is required.

You should be tech‑savvy, have a valid driver’s license and reliable vehicle, and be able to pass a background check.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; emphasis on heart and experience.
• Tech‑savvy with email, documents, spreadsheets, portals and video calls.
• Valid driver’s license and reliable vehicle; pass a background check and provide references.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
