The role emphasizes discharge planning, LOS management, and collaboration with care teams to optimize resource use while upholding NMH values. RN licensure in MN and case management experience are required.

Qualifications

• Bachelor's Degree from an accredited school of nursing.
• 3–5 years of acute care nursing experience.
• 1 year of direct case management experience.
• Hospital case management and utilization management experience.
• Knowledge of Medicare rules and regulations.
• Strong communication skills and ability to work with diverse teams.

Responsibilities

• Assess documented medical necessity of admitted patients to aid admission decisions.
• Provide clinically based information to support care coordination and discharge planning.
• Collaborate with nursing, social services, therapies and ancillary departments to ensure appropriate level of care across the continuum.
