Job summary

HealthPartners seeks an RN Care Manager at Methodist Hospital in Minnesota to deliver patient-centered care management across the Emergency Center and inpatient units.

The role partners with the care team to identify barriers to discharge, plan safe transitions, and apply nursing quality improvements daily. No direct supervisory responsibilities, but may supervise RN students and assist with orientation of new staff.

Qualifications

• Associate’s Degree in Nursing with commensurate experience.
• Minimum of one year hospital-based med-surg nursing experience.
• Current Minnesota RN license.

Responsibilities

• Provide care management services to patients in inpatient and Emergency Center settings.
• Identify barriers to discharge and implement appropriate transition plans with care teams.
• Collaborate with physicians, nurse practitioners, nurses, and other staff to coordinate patient care.
• No supervisory responsibilities; may supervise RN students or assist in orientation of new RN Care Managers.
