Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider (APP) to care for patients within the scope of training and the governing board. The APP will collaborate with other providers at a franchised AFC location, with duties and benefits varying by franchise.

Responsibilities include comprehensive health histories, diagnoses, therapy initiation, and patient education. Requires board certification and active PA/NP license in the state of practice, plus DEA and state-controlled

Qualifications

• Excellent communicator with staff, patients, and family.
• Able to multi-task and work independently.
• Active and current PA/NP licensure, DEA, and state substance certificate as required.

Responsibilities

• Perform complete, detailed medical histories and review records to develop assessments and order tests.
• Formulate diagnoses and refer patients to appropriate facilities or resources.
• Institute emergency measures and stabilize patients as needed.
