Job summary

American Family Care (AFC) is seeking an Advanced Practice Provider to deliver patient-centered care within franchise locations. The APP will work collaboratively with other providers and staff to ensure efficient patient flow and high satisfaction, following scope and agreements under governing boards.

Candidates must be board certified as PA or NP, hold active state licensure, DEA, and state-substance certificates, and be capable of independent work with strong communication skills.

Qualifications

• ANCC, AANP, or NCCPA Board Certified (ER/UC experience preferred)
• Active and current PA/NP licensure in the state of practice, DEA number, and state-controlled substance certificate
• Able to multi-task and work independently
• Excellent communicator with staff, patients, and family

Responsibilities

• Take complete health histories and review records; develop medical assessments; order labs, imaging, and diagnostics as appropriate
• Diagnose and institute therapy or referrals to appropriate facilities and resources
• Institute emergency measures and stabilization in emergencies
• Interpret patient data to determine status and management
• Provide guidance on healthcare and health promotion to patients and families
• Other duties as assigned
