Job summary

Wellmore Behavioral Health in Waterbury, CT is seeking a Case Manager to perform intake assessments, develop individualized care plans, and coordinate housing and supportive services for families referred by the Department of Children and Families. You will work with The Connection, Inc.

to monitor progress, maintain client records, and produce monthly reports. A Bachelor’s degree in a human services field and CPR/First Aid certification are required, along with reliable transportation and valid

Qualifications

• Bachelor's Degree in a human services-related field.
• CPR and First Aid Certification.
• Must have own reliable transportation and valid auto insurance.

Responsibilities

• Abides by the Code of Ethics of Wellmore Behavioral Health.
• Completes intake and client assessments to determine eligibility for inclusion in program.
• Develops an individualized plan of services for client, incorporating client input; reviews and revises as needed.
• Coordinates team review meetings with DCF social worker and other providers.
• Provides advocacy, crisis intervention and counseling services as needed.
• Maintains up-to-date records and submits reports to the case review team.
• Works with the DCF social worker to ensure safety and stability of children.
• Establishes and maintains housing placements with landlords; coordinates housing services.
