Job summary

The Center for Human Development (CHD) seeks an OWLS Unsheltered Outreach Caseworker to oversee cases in the Litchfield county area and provide outreach to unsheltered individuals, coordinating with support providers and adhering to regional network protocols.

This role requires extensive field work, a valid driver’s license, and a compassionate, client-centered approach. Competitive compensation and comprehensive benefits are offered.

Qualifications

• High school diploma or GED required.
• Two years’ experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations.
• Individuals with lived experience encouraged to apply.
• Bachelor’s degree in human services preferred.
• Comfortable being in the field and covering Litchfield county; not an office position and requires significant driving.
• Ability to utilize technology to perform essential job duties.
• Reliable, registered & insured vehicle and approved driving record check.
• Approved criminal background check (CORI).

Responsibilities

• Oversee a variety of cases in the Litchfield county area.
• Maintain a daily schedule for street outreach and coordination with external support providers.
• Work within the program model and protocols of Western CT Mental Health Network and Coordinated Access Network.
• Establish open communication, support and accountability among collaborating agencies.
