Job summary

The Center for Human Development (CHD) in Waterbury, CT is seeking an Outreach Caseworker to join our Residential Supports Program. The Outreach Caseworker oversees a variety of cases and provides case management for unsheltered individuals seeking housing in Waterbury county.

The role focuses on recovery-oriented, culturally sensitive support services to help clients with mental illness, substance use issues, and co-occurring disorders live independently in the community, with a driving focus

Qualifications

Experience working with psychiatric disorders or co-occurring population preferred
Bachelor’s degree from an accredited educational institution preferred
High school diploma or GED required
Reliable, registered & insured vehicle and approved driving record check
Approved criminal background check (CORI) required

Responsibilities

Oversee a variety of cases in Waterbury on a daily basis within the Residential Supports Program
Provide case management and daily supportive services to unsheltered individuals seeking housing
Support clients to live in the community as independently as possible within a recovery-oriented framework
Collaborate with staff to address mental health, substance use, and housing needs
