Job summary

Center for Human Development (CHD) seeks an Outreach Where you Live (OWLS) Unsheltered Outreach Caseworker to join our new program in the Litchfield county area in Torrington, CT. The role focuses on outreach to unsheltered individuals, connecting them with services and resources to end homelessness.

The position involves field work, driving throughout the county, and collaboration with regional networks. Includes a competitive benefits package and a full-time schedule.

Qualifications

• High school diploma or GED required.
• Two years’ experience with adults with severe mental illness, addiction/recovery or unsheltered populations.
• Comfort in the field; driving across Litchfield County.
• Reliable vehicle with insured status and CORI background check.

Responsibilities

• Oversee cases across venues in Litchfield County with daily street outreach.
• Provide tools and support to end homelessness and coordinate with providers.
• Maintain outreach schedule and liaise with Western CT Mental Health Network & Coordinated Access Network.
