Job summary

Center for Human Development (CHD) is seeking an Unsheltered Outreach Caseworker to join our SNOFO program serving the Litchfield County area. You will coordinate street outreach, resources, and external providers to assist unhoused individuals in reaching goals, aligning with CNOC protocols and local networks.

Qualifications

• High school diploma or GED required.
• Two years in a human service organization preferred.
• Two years’ experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations.
• Individuals with lived experience encouraged to apply.
• Bachelor’s degree from an accredited educational institution preferred.
• Registered & insured vehicle available for work purposes.
• Approved criminal background and driving check.

Responsibilities

• Oversee a variety of cases daily in the Litchfield, CT county area.
• Maintain a schedule including street outreach, equipment use, and coordination with external providers.
• Coordinate with Western CT Mental Health Network and Coordinated Access Network to ensure open communication and accountability.
