Job summary

Description

Communicate regularly with residents and their families about Plans of Care, PT, OT, and other treatment protocols. Attend and participate in morning meetings/stand-ups to facilitate communication with the team. Answer residents' questions regarding their care, treatment plans, illness progression, and other issues to ensure they feel safe and secure in our care. Monitor and adjust residents' statuses based on changing needs and conditions. Organize and prioritize daily work by assessing the needs of new, current, and discharging residents in your area of responsibility. Complete required documentation. Perform utilization review activities to provide residents with appropriate, timely, and cost-effective care. Coordinate care with residents, care providers, facility financial services, and third-party payers. Oversee all admissions and discharge activities. Coordinate referrals to and from our facility. Ability to relate positively, effectively, and appropriately with residents, families, staff, and professional colleagues. Ensure accurate charting and complete necessary paperwork promptly. Work independently and exercise sound judgment in interactions with physicians, providers, payers, residents, and their families. Communicate effectively to promote cooperation and collaboration among residents, families, caregivers, physicians, nurses, and other partners. Possess excellent time management skills to develop organized work processes in a high-volume environment with rapidly changing priorities. Have intermediate computer skills. Maintain professional, respectful, honest interactions with residents, families, staff, and partners.

Supervisory Requirements

This position has no supervisory responsibilities.

Qualifications

  • Education and/or Experience: Bachelor's Degree in Nursing or Social Work. Registered Nurse (RN) license preferred. Licensed (LVN or LPN) nurse acceptable. Two (2) years of clinical nursing experience preferred. Knowledge of Medicare, Medicaid, and benefits.
  • Language Skills: Ability to read technical procedures, policies, and respond effectively to questions from managers and employees.
  • Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions.
  • Reasoning Ability: Ability to solve practical problems, interpret instructions in various formats.
  • Certificates, Licenses, Registrations: LVN/LPN or RN license in the state of employment. Certification as a Case Manager (CCM) is a plus. Must maintain all required continuing education/licensing and remain in good standing with the Department of Public Health.

Physical Demands

Stand and/or walk frequently; sit occasionally; reach, push, pull frequently; talk and hear frequently; taste and smell frequently; lift up to 50 pounds frequently; climb, balance, stoop, kneel, crouch, crawl occasionally; possess good vision and focus ability.
