Qualifications

• Specialized Diploma graduate of an accredited nursing program.
• Minimum 3 years as a Registered Nurse in an acute care setting.
• Effective communication concepts and ability to communicate with patients and families.
• Patient- and Family-Centered Practice and cultural respect.
• Care Coordination across departments and facilities.
• Patient safety best practices and risk awareness.
• Medication administration and pharmacology knowledge.
• Discharge planning and arranging follow-up care.
• Health information documentation in medical records.

Responsibilities

• Collaborates with interprofessional teams to ensure consistent plans of care.
• Assesses patient needs and makes care recommendations considering age, psychosocial and cultural factors.
• Facilitates timely patient throughput from admission to discharge on the designated unit.
• Develops and maintains Smart Square schedules and on-call coverage as applicable.
• Monitors staffing levels and collaborates on payroll with Nurse Manager.
• Supports professional development and participates in governance programs and clinical ladder.
