Qualifikationen

• Specialized Diploma Graduate of an accredited school of nursing required.
• 3 years as a Registered Nurse in an acute care setting.
• Effective communication concepts and partnership with patients and families.
• Knowledge of patient-centered practice and cultural awareness.
• Care coordination across multidisciplinary teams.
• Patient safety practices and risk reduction.
• Medication administration and prescription knowledge.
• Discharge planning and arranging follow-up care.
• Assessment and nursing diagnosis with clinical judgement.
• Health information documentation in written or electronic form.

Aufgaben

• Interact with coworkers, visitors, and staff upholding Jefferson Health values.
• Provide safe, compassionate, and coordinated patient care respecting patient preferences.
• Collaborate with interprofessional teams to align care plans.
• Assess patient needs and make care recommendations across the unit.
• Facilitate patient throughput and discharge optimization on the unit.
• Assist in plan of care rounds as protocol requires.
• Prepare smart square schedules and on-call scheduling as applicable.
• Resolve staff, patient, and physician issues and complaints promptly.
• Monitor staffing to meet patient care needs and approve overtime when needed.
• Support Nurse Manager with payroll adjustments and performance evaluations.
• Contribute to clinical practice changes and quality improvement initiatives.
