Qualifications

• 3 years as a Registered Nurse in an acute care setting.
• Effective Communication- Understanding of effective communication concepts, tools and techniques.
• Active RN license required in state of practice.
• Cultural Awareness and patient-family centered practice.
• Care Coordination and discharge planning knowledge.
• Health Information Documentation and clinical judgment.

Responsibilities

• Lead daily operations of the unit and mentor colleagues.
• Coordinate patient care delivery and flow through the continuum of care.
• Collaborate with medical staff to ensure consistency of interdisciplinary plans of care.
• Assist plan of care rounds and manage staffing changes as needed.
• Provide patient care support and uphold quality standards.
