Zusammenfassung

HealthRIGHT 360, a nonprofit provider of integrated behavioral health and residential services, seeks a Case Manager for the Prototypes Women’s Center in Pomona. You will coordinate intake, assist with Medi-Cal applications, and connect clients to housing, employment, and community resources across outpatient, residential, or field settings.

You will work within a multidisciplinary team to ensure treatment plans are followed and progress is documented within HIPAA-compliant systems.

Qualifikationen

• High school diploma or equivalent required.
• SUD Counselor registration (CCAPP/CAADE/CADTP) required or obtainable.
• Bachelor’s degree in Social Work, Psychology, or related field preferred.
• First Aid and CPR certification preferred; must obtain if not on hire.
• Valid California Driver’s License and access to insured transportation.

Aufgaben

• Assist intake by completing case management assessment and entering financial and benefit information.
• Support clients in applying for Medi-Cal or transferring Medi-Cal County.
• Assess client needs and complete all releases of information.
• Connect clients to benefits, health services, housing, and community resources.
• Coordinate scheduling, communication with probation, DCFS, and treatment providers.
• Deliver services in outpatient, residential, or field settings per assignment.
• Prepare clients for job search with resume writing and prep.
• Monitor progress toward treatment plan goals and provide input.
• Seek clinical consultation as needed and collaborate with the treatment team.
• Transport clients as needed and document UA test results.
