Job summary

HealthRIGHT 360 is seeking an Intake Coordinator II at its Prototypes Women’s Center in Pomona, CA. The role begins with a warm, in-person intake to determine SUD service needs, collect admissions forms, and craft initial treatment plans.

You will coordinate with counselors, case managers, and other staff to stabilize new clients and address urgent needs. The position emphasizes collaboration, timely documentation, and warm handoffs to residential treatment teams while ensuring HIPAA and program

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, Marriage and Family Therapy, or related field from an accredited graduate school.
• Valid registration or licensure from the California Board of Behavioral Sciences as an AMFT, ACSW, APCC, LMFT, LPCC, or LCSW.
• Valid First Aid and CPR certification or ability to obtain within 30 days of hire.

Responsibilities

• Provide comprehensive initial assessment of candidates’ needs.

Education

Master’s degree in Psychology or Counseling or Social Work or related field

CA Board of Behavioral Sciences license/registration (AMFT/ACSW/APCC/LMFT/LPCC/LCSW)

First Aid and CPR certification

Experience with intake assessments

Experience with Medi-Cal and County funding processes
