Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. In this role, you’ll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes.

This part-time, day-shift position offers day-one benefits and a collaborative environment. You’ll work with providers, care managers, pharmacists, and others to manage triage by phone and in-office visits,

Qualifications

• Graduate of an accredited Registered Nurse program.
• Active RN license in Michigan with CPR certification.
• 1-2 years of clinical experience in a medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education.
• Coordinate care and transitions of care post-discharge.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and others.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
