Job summary

Upham's Community Care is seeking a Triage Registered Nurse for its Primary Care department in Boston. The RN will triage calls, assess patient needs, and guide care pathways following clinic protocols.

The ideal candidate has MA RN licensure, at least two years' nursing experience (urgent care/triage preferred) and strong communication skills. Bilingual Spanish or Cape Verdean Creole is preferred, with Epic EMR familiarity or willingness to learn.

Qualifications

• RN license in Massachusetts required.
• Minimum two years of nursing experience, preferably urgent care or triage.
• Experience in FQHC, community health center, or similar setting preferred.
• Bilingual in Spanish or Cape Verdean Creole preferred.
• Familiarity with Epic EMR or willingness to learn.
• Strong communication and teamwork skills.

Responsibilities

• Triage calls, assess patient needs, and prioritize urgent matters.
• Document guidance in nursing notes and Epic EMR.
• Schedule appointments and coordinate with providers.
• Educate patients and provide health education per protocols.
• Assist with lab results review and follow-up.
• Support cross-training and participate in interdisciplinary team.
