Job summary

Family Health Center in Worcester, MA is seeking a qualified RN Triage Nurse for our Walk-In Center. The role focuses on providing nursing care within a family practice/urgent care setting, following evidence-based triage protocols and coordinating care as needed.

You will triage patients, provide health guidance, arrange access to interpreter services when needed, document in the EHR, and schedule next steps including Primary Care, Same Day/Urgent Care, or ER as appropriate.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Massachusetts RN license required.
• BLS certification required.
• Two years nursing experience, with at least one year of telephone triage in ambulatory/family practice setting preferred.
• Experience with EMR environment required.

Responsibilities

• Triage patients using evidence-based protocols; provide triage, education, and self-management support as needed.
• Provide health care advice and coordinate services with the care team.
• Document care plans and patient understanding in the EHR; follow documentation policy.
