Job summary

Pathways Healthcare, a leading home health and hospice provider, is seeking a Triage Registered Nurse - Hospice for weekend shifts in NH service areas including Salem, Londonderry, and surrounding towns.

Base compensation is $600 per weekend plus $50 per visit and $100 per admission, with mileage reimbursement and 401(k) matching. Part-time weekend schedule Saturday-Sunday 8:30 am–8:00 pm; EMR proficiency and strong bedside manner required.

Qualifications

• 2+ years nursing experience, geriatrics desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient status changes.
• Responds promptly to afterhours calls (weekends 8:30 AM to 8:00 PM).
• Assesses patient needs through triaging and in-home care.
• Collaborates with physicians, nurse practitioners, and the team on the Plan of Care.
• Responds to inquiries from patients, caregivers, and medical team.
• Consults patients and caregivers before, during and after treatment.

Job Description:

• Role: Triage Registered Nurse - Hospice
• Rate: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100)
• Service Area: Salem, Atkinson, Hampstead, Londonderry, Windham, Manchester, Nashua, Bedford, Goffstown, Litchfield, Amherst, Milford, Hudson, Pelham, Chester, Auburn, Derry areas.

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

What We Offer:

• Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr.
