Job summary

Pathways Healthcare in Manchester, NH, seeks a dedicated Registered Nurse - Hospice to join our physician- and nurse-led home health team. You will assess patients, write plans of care, and coordinate with families to deliver compassionate, patient-centered care in home settings.

You will operate in our multi-town service area, Monday–Friday, with on-call duties. We value clinical judgment, excellent communication, and compliance with health care laws and documentation accuracy while supporting

Qualifications

  • 2+ years nursing experience, preferably in geriatrics is desirable.
  • New nurses welcome; strong training program offered.
  • Degree in Nursing from an accredited institution.
  • Proficient with electronic medical records (EMR).
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Valid driver's license and own insured vehicle.

Responsibilities

  • Assess and evaluate patient status and needs.
  • Write and initiate plan of care; update as needed.
  • Regularly re-evaluate patient and family needs.
  • Develop care plans based on nursing diagnoses and goals.
  • Administer medications and treatments as prescribed.
  • Document notes and changes in patient condition.
  • Communicate with physicians regarding patient needs.

Job Description:
  • Role: Registered Nurse - Hospice
  • Service Area: Manchester, Goffstown, Bedford, Hooksett, Concord, Bow, Suncook, Allenstown, Pembroke areas.
  • Schedule: Monday – Friday (8:30-5:00pm)
  • Job Type: Full-time

Responsibilities:
  • Assesses and evaluates patient’s status by:
  • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Provides and maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.
Additional Duties:
  • Participates in on-call duties.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities.
