Responsibilities

• Provide telephonic nursing care using triage tools.
• Identify emergent situations and transfer calls per protocol.
• Communicate information to physicians as needed.
• Conduct thorough nursing assessments of presenting symptoms.
• Develop, implement, and evaluate care plans.
• Prioritize interventions to reduce adverse outcomes.
• Schedule follow-up appointments.
• Return calls promptly.
• Maintain patient confidentiality per HIPAA.
• Document inquiries clearly and immediately after calls.
• Promote Tribe Way and maintain professional interactions.
• Attend business-hour meetings with Clinical and Triage teams.
