Job summary

Qualifications

• Assess patients' medical conditions via phone or in-person consultations.
• Prioritize treatment based on severity and medical history.
• Document patient interactions and maintain medical records.
• Provide medical advice within RN/LPN scope.
• Coordinate referrals with healthcare providers.
• Perform basic nursing duties such as wound care, administering medications, and monitoring vital signs.
• Educate patients on self-care practices and preventative health measures.
• Manage patient follow-ups and continuity of care.
• Assist in maintaining a clean and safe healthcare environment.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
