Job summary

Apploi is seeking an LPN Patient Navigator to support care coordination and triage for patients. Under nursing supervision, you will assess health concerns and guide dispositions to appropriate care settings, while supporting providers in triaging walk-in patients in a fast-paced clinic.

The role emphasizes community health orientation; CPR is required, with experience in rural or community health settings preferred. Full-time hours in a clinic environment with a strong team approach.

Qualifications

• Minimum 2 years healthcare experience; CNA/Med Aide/MA experience preferred.
• CPR certification required.
• Experience with triage or care coordination preferred.

Responsibilities

• Coordinate care by triaging walk-in patients and scheduling provider appointments.
• Assess patient health needs using established triage protocols.
• Document symptoms, advice, and patient responses in EHR accurately.
• Obtain provider input to determine best clinical course of action.
• Maintain and follow policies to improve access to care and outcomes.
