Job summary

Heartland Health Services - Central Illinois is seeking a Licensed Practical Nurse (LPN) to provide phone-based triage and nursing guidance to patients across multiple clinic sites. You will assess symptoms, determine appropriate care levels, and collaborate with RN and physician teams.

Full-time role with 4x10-hour shifts possible, competitive hourly pay and benefits starting day one, plus a 4% 401(k) match and four weeks of PTO annually. Occasional hybrid option may be available.

Qualifications

• Current LPN license required.
• Effective communication with patients by phone.
• Experience with telephone triage or clinical call handling preferred.
• Proficiency with basic computer tools (EMR, Word, Excel).

Responsibilities

• Triage calls and assess symptoms to determine care level.
• Process medication refills and review lab results per protocol.
• Document in EMR and coordinate with RN/physician teams.
• Schedule appointments or referrals as needed.
