Texana Center in Brookshire, TX is hiring a Care Coordinator to deliver direct services to youth and adults with chronic mental illness, in community settings and on-site. You will assess, plan, coordinate, monitor, and link patients to community resources, while building skills and providing crisis intervention.

This full-time role follows a typical 8:00 AM–5:00 PM schedule, with occasional after-hours flexibility.

Formación

• Bachelor's or advanced degree in a relevant field (counseling, psychology, human services, nursing, social work, etc.).
• Texas driver's license in good standing.

Responsabilidades

• Provide direct services to youth and adult individuals with chronic mental illness in community and on-site settings.
• Offer skills building, psychosocial rehab and crisis intervention services.
• Develop recovery plans and complete ANSA or CANS for patients on the caseload.
• Ensure documentation and assessments meet applicable standards and performance benchmarks.
• Collaborate within a multi-disciplinary treatment team and maintain engagement, time management, and organization.
• Work a routine 8:00 AM–5:00 PM schedule with potential after-hours flexibility.
