Pine Grove Manor in Missouri seeks a registered nurse to serve as the wound care coordinator, overseeing the center's skin management program and guiding clinical practice. You will lead weekly wound team rounds with interdisciplinary care planning, document treatment recommendations, monitor Braden risk scores on admission, and implement preventive interventions to reduce pressure injuries.

Candidates must hold an active RN, LVN, or LPN license in the state, with at least one year of nursing

• Currently licensed as RN/LVN/LPN in state.
• One-year prior nursing management experience preferred.

• Identify and maintain effective methods to gather pressure ulcer and skin alteration related information
• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation
• Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners
• Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions

Objective

A registered licensed nurse designated to act as the coordinator of the center’s wound team to oversee the general application of the skin management program. The following activities are performed as directed or clinically indicated.

Principal Responsibilities
