Job summary

Apploi seeks a registered nurse to serve as the wound care coordinator of the center’s wound team. The role oversees the skin management program, coordinates weekly rounds, and ensures appropriate documentation and physician orders are followed.

The candidate will explain and document Braden risk assessments, implement preventive skin interventions, and collaborate with the care team to reduce pressure ulcers and skin alterations.

Qualifications

• Currently licensed as RN/LVN/LPN in state.

Responsibilities

• Identify and gather pressure ulcer and skin alteration information.
• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation.
• Collect, analyse and provide pressure ulcer information to nursing staff and other healthcare practitioners.
• Investigate and identify centre practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions.

Principal Responsibilities

• Identify and maintain effective methods to gather pressure ulcer and skin alteration related information
• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation
• Collect, analyse and provide pressure ulcer information to nursing staff and other healthcare practitioners
• Investigate and identify centre practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions

Qualifications

• Currently licensed as RN/LVN/LPN in state

One-year prior nursing management experience preferred
