Job summary

Bluebird Wellness & Rehabilitation is seeking a registered nurse to serve as the coordinator of the center’s wound team, overseeing the skin management program and guiding clinical care. The role emphasizes leadership, assessment, and collaboration with the interdisciplinary team.

• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation
• Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners
• Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions

Objective
