Job summary

OPCO Skilled Management seeks a licensed registered nurse to act as the coordinator of the center’s wound team, overseeing the skin management program. You will lead weekly rounds, coordinate interdisciplinary care plans, and ensure proper documentation.

The role emphasizes gathering and analyzing pressure ulcer data, guiding preventive interventions, and ensuring Braden risk assessments are completed on admission and throughout care.

Qualifications

• Currently licensed as RN/LVN/LPN in state.
• One-year prior nursing management experience preferred.

Responsibilities

• Identify and implement methods to gather pressure ulcer and skin alteration information.
• Coordinate and lead weekly wound team rounds and care planning, including documentation.
• Collect, analyze and report pressure ulcer information to nursing staff and practitioners.
• Investigate center practices and risk-minimizing measures, including Braden risk scales on admission and preventive interventions.
