Overview

Location: East Valley

Role

Treatment Coordinator: The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing service needs, including primary care and behavioral health. This position helps clients and families obtain community support, identify resources, and resolve barriers.

Responsibilities

• Coordinate services for children and adults according to the treatment plan.
• Respond to initial crisis calls, identify needs, and allocate necessary resources.
• Engage formal and informal resources for the client, including transition, discharge, and aftercare plans for behavioral health services, demonstrating teamwork and shared leadership.
• Coordinate behavioral health services with agencies such as DCS, Probation/Parole, DDD, AZ Long Term Care, schools, and other providers.

Qualifications

• Bachelor's Degree in a behavioral health field preferred; experience and education may be combined.
• Relevant experience in a behavioral health setting.
• Level 1 Fingerprint Clearance Card.
• Verification of education, employment, and references.
• Valid AZ Driver's License, auto insurance, and compliance with JFCS driving guidelines.
