Location: East Valley Healthcare Center in Gilbert, AZ

Overview

The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing service needs, including primary care and behavioral health. This position helps clients and families obtain community support, identify resources, and resolve barriers.

Responsibilities

1. Coordinate services for children and adults according to the treatment plan.
2. Respond to initial crisis calls and identify needs and resources.
3. Engage formal and informal resources for the client, including transition, discharge, and aftercare plans of behavioral health services, demonstrating teamwork and shared leadership.
4. Coordinate behavioral health services with other agencies such as DCS, Probation/Parole, DDD, and schools.

Qualifications

1. Bachelor's Degree in behavioral health preferred; experience and education may be combined.
2. Relevant experience in a behavioral health setting.
3. Level 1 Fingerprint Clearance Card.
4. Verification of education, employment, and references.
5. Valid AZ Driver's License, auto insurance, and compliance with driving guidelines.
