FreedomCare LLC is seeking a Case Manager, RN to conduct in-home assessments and manage care for patients in the Portland, OR area. This field-based role requires regular travel to patients' homes and coordination with PCAs and interdisciplinary teams.

Ideal candidates hold an Oregon RN license and a nursing diploma, with strong skills in care coordination, documentation, and patient education. New graduates are welcome to apply.

Qualifikationen

• Diploma from an accredited nursing program. New graduates welcome!
• Oregon RN license is required.
• Ability to travel within assigned geographic area by car.
• Proficient with Case Management Systems and MS Office.

Aufgaben

• Conduct initial in-person clinical assessments and coordinate service activation.
• Develop, implement, and manage patient activity plans.
• Document changes and complete client records timely and accurately.
• Coordinate care with physicians, PCAs, and interdisciplinary care teams.
• Provide ongoing coaching and connect families to appropriate resources.
• Ensure compliance with regulatory standards and company policies.
• Travel within assigned geographic area to conduct in-home visits.
