Job summary

FreedomCare is seeking a Case Manager, RN for our Ohio team to conduct in-home assessments, orient new patients and caregivers, and provide ongoing care management and support. The role requires frequent travel to patients' homes in Youngstown, OH.

As part of a collaborative care team, you will assess needs, develop individualized care plans, educate patients and PCAs, and ensure high-quality, compliant services with timely communication to the broader care network.

Qualifications

• Diploma from an accreditted nursing program. New graduates welcome!
• Ability to articulate cases through effective summaries.
• Ability and flexibility to travel within assigned geographic area.
• Valid driver’s license and car insurance required.
• Proficient in common software and web-based applications (Case Management Systems, MS Office).
• Bilingual is a plus.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation.
• Develops, implements, manages patient activity plan(s) and revisions.
• Completes all client documentation and notes following visits.
• Coordinates care with physicians and other disciplines involved in the client’s care.
• Provides ongoing coaching to drive utilization of best practices and PCA education.
• Reviews PCA notes and addresses changes in condition or environment.
• Ensures compliance with regulatory standards and company policies.
• Performs other duties as directed by supervisor.
