PeaceHealth is seeking an RN Home Care Case Manager within Home Health for a full-time, day-shift role in Oregon. You will deliver direct skilled nursing care and coordinate management for patients in home care and clinic settings.

The position requires an active RN license in Oregon, at least 2 years of acute or home health experience, and adherence to regulatory guidelines. Travel via personal vehicle may be involved, with a comprehensive benefits package and collaborative team environment.

Qualificações

• Licensed Registered Nurse in state of practice (Oregon)
• BSN preferred
• Minimum 2 years of acute clinical or home health/hospice nursing experience
• Home health experience preferred or ability to orient to competency
• Must meet age-specific growth and development needs
• Driver's license and auto insurance proof
• Basic Life Support required upon hire

Responsabilidades

• Conduct initial patient/home evaluation and assess admission eligibility for home health/hospice services
• Provide skilled nursing care and education to patients and caregivers per the treatment plan
• Develop, coordinate, modify and manage the written plan of care with physician oversight
• Coordinate care with interdisciplinary team to ensure smooth care continuum
• Execute discharge planning aligned with patient goals and outcomes
• Maintain timely clinical documentation per guidelines
• Assist with family Bereavement services planning for patient death
• Perform other duties as assigned
