Job summary

Your Health is seeking a Registered Nurse to service patients in the 29223 and 29224 area. The Visiting Nurse (Nurse Case Manager) will travel to patients' homes and facilities to evaluate care needs and initiate telemedicine visits.

The role requires reliable transportation, working 8-hour shifts Monday–Friday (8am–5pm), and collaboration with care teams to coordinate, document, and adjust Care Management plans.

Qualifications

• Must be a licensed nurse.
• Three years of clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage multiple responsibilities with sound judgment.
• Must be able to travel between offices and facilities.
• Licensed driver with insured vehicle.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records and enroll patients in Care Management.
• Initiate a Care Management Plan of Care for eligible patients.
• Capture all diagnoses at highest specificity and close gaps.
• Complete AWVs to be reviewed by the provider.
• Complete cognitive impairment screenings.
• Complete SDoH assessments and/or screenings.
• Complete ACPs to be reviewed with the patient by the provider.
• Evaluate for home health, hospice, palliative, or RPM/DME as appropriate.
• Coordinate care and reconcile prescribed medications.
• Provide post-discharge education.
• On call may be required.
