Your Health is seeking a Registered Nurse to serve patients across the [LOCATION] area as a Visiting Nurse / Nurse Case Manager. The role requires traveling to patients’ homes and facilities to assess needs, initiate telemedicine visits, and coordinate with the care team.

Full-time, 8-hour shifts, Monday–Friday, 8am–5pm, with compensation and benefits described. We are a leading physician group serving South Carolina and Georgia, delivering care in homes, clinics, and care facilities.

Formación

• Must be a licensed nurse; RN preferred; license in good standing.
• Minimum of three (3) years clinical experience.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Must be able to travel between offices, facilities, etc. and be a licensed driver.

Responsabilidades

• Facilitate receiving all medical records from patient’s primary provider and specialists.
• Review medical records.
• Enroll patients in Care Management, if eligible.
• Initiate a Care Management Plan of Care, if the patient is eligible.
• Capture all diagnoses at the highest specificity and ensure gaps are closed.
• Complete AWVs and cognitive impairment screenings.
• Complete Social Determinants of Health assessments.
• Complete ACPs to be reviewed with the patient by the provider.
• Evaluate for home health, hospice, palliative, or consults with Your Health Specialty Division.
• Evaluate for RPM devices, resources, or tools to improve quality of life.
• Communicate and coordinate care; document Care Management activities.
• On call may be required.
