Job summary

RWJBarnabas Health in Somerset, NJ is seeking a Nurse Care Coordinator to join the ACO team. In this salaried, day-shift role you will review patient records, coordinate care across disciplines, and support transitions to appropriate levels of care to improve outcomes.

Responsibilities include building individualized care plans, collaborating with physicians, nurses, social workers and payers, documenting activities, and contributing to performance improvement initiatives.

Qualifications

• Bachelor's nursing degree from an accredited school.
• Current unrestricted New Jersey nursing license with multi-state eligibility.
• Experience in Utilization Management, Case Management, Disease Management, Care Coordination or Discharge Planning.
• Proficiency in Epic EHR.
• Excellent communication and interpersonal skills with diverse populations.
• Outcomes-driven with ability to meet performance targets.
• Flexibility to adapt to changing workflows and patient care needs.

Responsibilities

• Review patients to determine medical necessity and enrollment in care management.
• Develop care plans and monitor progress toward goals.
• Coordinate care for patients with complex needs across settings.
• Collaborate with physicians, nursing, social work and payers to eliminate barriers.
• Document clinical activities in the medical record and support accurate documentation.
• Adapt to evolving priorities and participate in performance improvement.
