Job summary

Signify Health, Inc. seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations, traveling to members’ homes across a defined coverage area.

Visits last about 45 minutes and include medical history review, physical exam, and potential diagnostic tests as ordered by the health plan. The role requires active licenses in the coverage areas, board certification, and the ability to obtain multi-state licenses.

Qualifications

• Active, unrestricted licenses in the coverage area(s).
• Board certification required.
• Ability to obtain and maintain multi-state licenses as required.
• Ability to cover all counties within state and flex to other areas of business need.

Responsibilities

• Bring your clinical expertise to in-home visits, including comprehensive health evaluations.
• Travel locally and across states as required by coverage area.
• Conduct virtual visits when needed.
• Perform diagnostic and preventive services as indicated.
• Use Signify Health services to support member health outcomes.
