Job summary

Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to deliver in-home health evaluations across a defined coverage area. Travel up to 100% may be required, with overnight stays and licenses in multiple states as dictated by business needs.

You will conduct visits lasting about 45 minutes, review medications and medical history, perform physical exams, and provide diagnostic and preventive services as ordered by health plans.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Able to obtain and maintain multi-state licenses as needed.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Conduct in-home health evaluations with patient-focused approach.
• Perform medication and medical history review and physical evaluation.
• Coordinate with health plans and clinicians across states.
• Travel locally and across licensed states up to 100% of the time.
• Utilize iPad and clinical workflow for evaluations; no prescribing required.
• Provide indicated diagnostic and preventive services as needed.
