Job summary

Signify Health, Inc. seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home visits for health plan members. You will conduct health assessments, review medications and medical history, and perform physical evaluations using an iPad.

Travel locally within licensed states is required, with potential virtual visits as needed. The role requires active, unrestricted NP/PA licenses with board certification; multi-state licenses are a plus.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification is required.
• Multi-state licenses are preferred.
• Willingness to travel locally within licensed states.

Responsibilities

• Conduct in-home health visits for members, including history review and physical evaluation.
• Provide diagnostic and preventive services as indicated by health plan.
• Use iPad-based clinical workflow; no need to prescribe or alter existing treatment.
• Travel locally within licensed states; virtual visits as needed.
