Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You will travel up to 100% of the time within a defined coverage area, using an iPad workflow, with no requirement to prescribe medications or order labs.

The role emphasizes building trusted relationships, performing 45‑minute visits, and delivering comprehensive health evaluations at patients\' doors across the region.

Qualifications

• Active, unrestricted NP or PA license in at least one coverage area.
• Board certification is required.
• Ability to obtain and maintain multi-state licenses as needed.
• Willingness to travel across a defined coverage area.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Be flexible to travel locally and within licensed states
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
