Job summary

GuideStar Eldercare seeks a Nurse Practitioner to deliver private-practice-style care within nursing homes and assisted living facilities. You will operate with clinical autonomy, supported by corporate leadership and in-house admin teams, while managing a defined patient caseload.

As part of a multidisciplinary team, you will travel daily from home, perform initial evaluations, ongoing therapy, and cognitive testing, and document services for each patient.

Qualifications

• Proof of State licensure in Tennessee.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.
• COVID-19 and flu vaccination as required by GuideStar Eldercare policy.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations and continuing therapy services, including cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
