Job summary

GuideStar Eldercare is seeking a Nurse Practitioner to address dementia and other neurocognitive disorders in nursing homes and memory care facilities. You will join a multidisciplinary team dedicated to patient safety, functionality, and dignity.

You will travel daily from home to assigned facilities, perform initial evaluations, provide ongoing therapy, and conduct cognitive testing, while documenting needs and maintaining strong relationships with facilities and families.

Qualifications

• Licensed as a Nurse Practitioner in Indiana.
• Ability to provide initial evaluations and cognitive testing.
• Experience delivering therapy services to neurocognitive patients.

Responsibilities

• Travel daily to assigned facilities from home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
