Job summary

GuideStar Eldercare in Indiana is seeking a Nurse Practitioner to address dementia and other neurocognitive disorders in nursing homes and assisted living facilities. You will manage a private-practice-like caseload with support from corporate clinical leadership and in-house scheduling, coding, and billing.

As part of a multidisciplinary team, you will travel daily to facilities, perform evaluations, deliver therapy services, document needs, and maintain relationships with clients and families,

Qualifications

• Proof of Nurse Practitioner licensure.
• Ability to work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
