Guidestar Eldercare is seeking a Nurse Practitioner to address the needs of aging populations in nursing homes and assisted living facilities. You will enjoy private-practice autonomy with caseloads and support from corporate clinical leadership, scheduling, coding, and billing teams.

As part of a multidisciplinary team focused on safety, functionality and dignity, you will travel daily from home to assigned facilities, perform initial evaluations, cognitive testing, and ongoing therapy

Qualifications

• Proof of State licensure.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
