Job summary

GuideStar Eldercare seeks a Nurse Practitioner to address dementia and neurocognitive disorders in nursing homes and assisted living facilities. You will travel daily from home, perform initial evaluations, ongoing therapy, and cognitive testing, and document services for each patient.

You will build strong relationships with facilities, patients, and families while supported by corporate clinical leadership and in-house scheduling, coding, and billing teams.

Qualifications

• Proof of State licensure.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
