GuideStar Eldercare is seeking a Nurse Practitioner to address dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. You will enjoy autonomy similar to private practice, with an assigned caseload and support from corporate clinical leadership.

As part of a multidisciplinary team focused on patient safety, functionality and dignity, you will travel daily from home to facilities to perform initial evaluations, ongoing therapy services,

Qualifications

• Proof of State licensure.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
